چکیده

We hypothesized that N-methyl-d-aspartate (NMDA) receptors mediate some or all of the capacity of inhaled anesthetics to prevent movement in the face of noxious stimulation, and that this capacity to prevent movement correlates directly with the in vitro capacity of such anesthetics to block the NMDA receptor. To test this hypothesis, we measured the effect of IV infusion of the NMDA blockers dizocilpine (MK-801) and (R)-4-(3phosphonopropyl) piperazine-2-carboxylic acid (CPP) to decrease the MAC (the minimum alveolar concentration of anesthetic that prevents movement in 50% of subjects given a noxious stimulation) of 8 conventional anesthetics (cyclopropane, desflurane, enflurane, halothane, isoflurane, nitrous oxide, sevoflurane, and xenon) and 8 aromatic compounds (benzene, fluorobenzene, o-difluorobenzene, p-difluorobenzene, 1,2,4trifluorobenzene, 1,3,5-trifluorobenzene, pentafluorobenzene, and hexafluorobenzene) and, for comparison, etomidate. We postulated that MK-801 or CPP infusions would decrease MAC in inverse proportion to the in vitro capacity of these anesthetics to block the NMDA receptor. This notion proved correct for the aromatic inhaled anesthetics, but not for the conventional anesthetics. At the greatest infusion of MK-801 (32 g · kg · min) the MACs of conventional anesthetics decreased by 59.4 3.4% (mean sd) and at 8 g · kg · min by 45.5 4.2%, a decrease not significantly different from a 51.4 19.0% decrease produced in the EC50 for etomidate, an anesthetic that acts solely by enhancing -amino butyric acid (GABA) receptors. We conclude that some aromatic anesthetics may produce immobility in the face of noxious stimulation by blocking the action of glutamate on NMDA receptors but that conventional inhaled anesthetics do not.

Lipid Emulsion for Local Anesthetic Systemic Toxicity

The accidental overdose of local anesthetics may prove fatal. The commonly used amide local anesthetics have varying adverse effects on the myocardium, and beyond a certain dose all are capable of causing death. Local anesthetics are the most frequently used drugs amongst anesthetists and although uncommon, local anaesthetic systemic toxicity accounts for a high proportion of mortality, with lo...

Key questions This tutorial reviews the basic pharmacology of common intravenous (IV) anaesthetic drugs. By the end of the tutorial, you should be able to decide on the most appropriate drug to use in the situations below and for what reason: • A patient with intestinal obstruction requires an emergency laparotomy. • A patient with a history of throat cancer, showing marked stridor and signs of...

Objective- The study aims to determine efficacy of propofol as an inmersión agent to induce anesthesia in rainbow trout (Oncorhynchus mykiss). Design- Experimental study. Animals- 36 healthy rainbow trout Procedure- Trouts were sorted ramdomly in two groups, 18 fish each one. Both groups were anesthesized by bath, one of them with 2,5 mg/l, the other one at 5 mg/l concentratio...

Orphan anesthesia: an initiative of the scientific working group of pediatric anesthesia of the German society of anesthesiology.

Anesthesia is often described as the best example of applied pharmacology. Indeed you probably cannot be a good anesthetist if you do not have a good grasp of pharmacology. A key aspect to a good anesthetic is the skill of exploiting the favorable pharmacodynamic interactions between agents. This superbly written and easily understood review by Hannan and Anderson helps demystify the complex sc...

Local Anaesthetic Pharmacology 11 / 07 / 05

Local anaesthetic drugs are used widely for the provision of anaesthesia and analgesia both intraand post-operatively. Understanding the pharmacology of these agents as a group, as well as the differences between specific drugs, enables the anaesthetist to use them safely to maximum effect. This tutorial focuses on the basic structure and function of local anaesthetics. Learning will be improve...
